Participant Waiver & Agreement *

AXS Wellness LLC / First Responder Sleep Recovery Participant Waiver and Agreement I agree not to film, record, or distribute any part of this training without express permission from the founder, Jacqueline Toomey. I understand this program is educational and not a substitute for medical advice. I am responsible for consulting my physician before participating in any physical exercises, stretches, somatic movements, or breathwork. I acknowledge these activities involve physical exertion and may carry risk of injury. I take full responsibility for my participation and any resulting injuries or health issues. I will inform the instructor of any medical conditions, injuries, or pregnancy prior to participation. If I am pregnant, I understand I participate at my own risk. In consideration of being allowed to participate, I voluntarily assume all risks and agree to hold harmless AXS Wellness LLC / First Responder Sleep Recovery, its instructors, and affiliates from any and all liability, including for injury or damage, whether known or unknown, even if caused by negligence. I understand that a 72-hour notice is required to transfer my registration to another date. I accept full responsibility for any loss or damage to personal property during participation. I have read and understand this waiver and agree to its terms voluntarily.
